Split lines into paragraphs (§4.1).
A new paragraph begins at a blank line, a leader-only line, a change
of prefix, a change of indent, or a list marker. Blank, leader-only
and fenced runs come back as fillable: false so they survive the
round trip verbatim — carrying them in the same list as the fillable
runs is what makes it impossible to drop them by forgetting a branch.
