4.4.h: cached decoded textDocument/semanticTokens/full
response for one buffer. Same shape as the other LSP
per-buffer caches: keyed on (BufferId, document_version),
invalidated by the pump when the version changes.
result_id is the server-issued tag the host sends back
on the next full/delta request so the server knows which
baseline to diff against (4.4.i).
raw_data (4.4.i) keeps the un-decoded Vec<SemanticToken>
alongside the decoded view so delta edits can splice into
it before the host re-decodes for the renderer.
